I have been using this system for at least 15 yrs. It is the BEST on the market as it comes with an outer casing. The cold stays actually for at least an hour maybe a little more. If you have low back spasms--my recommendations are to get the x-long as it will also decrease the swelling across your buttocks as well. It's better than some meds at times...I use it sparingly as I want it to work when I REALLY need it. Good luck.
